Hello Nejc,

You can find the mappings you're looking for in the kg4ToKg5 table via 
the table browser (http://genome.ucsc.edu/cgi-bin/hgTables). It is a 
little tricky as this table doesn't show up within the shortlist of 
tables for the UCSC genes tracks. Within hg19 select:

group: All Tables
database: hg19
table: kg4ToKg5

Or to directly download the whole table file you can grab it from our 
downloads server here:

http://hgdownload.cse.ucsc.edu/goldenPath/hg19/database/

(scroll down to the "kg4ToKg5.*" files)
