I just checked in a modification to Portage
(<http://bugs.gentoo.org/show_bug.cgi?id=4709> for details) which
supports this very thing. It keeps copies of installed config files so
that it can compare against them when updating, and even supports
auto-merging (ala CVS). I've been using it for close to a week now and
love it. I just updated to the latest version of baselayout, and only
had to run etc-update on one file (rc.conf).

Hopefully it won't take too long to get approved (certainly not for
1.4, I'm sure); since I've moved all of the ._cfg files to a structure
under /var/cache/edb I wouldn't be surprised if there's a certain
amount of resistance to the change. I think it's a worthwhile addition,
though.
