USE="-gnome" emerge xmms
Would emerge it without the older gnome stuff.

Should I just set -gnome in make.conf to ensure other packages don't pull in older stuff? What do I stand to lose if I do that?
You could. xmms is the only program I can think of that wants to pull in gnome 1.4 stuff. You may stand to lose some functionality in certain packages, and some might not put launchers in your gnome menu when you merge them.
It's easier to just catch xmms when it wants to update and do it manually.
