IIRC, portage will break in subtle ways if a package in is the world file 
but not actually installed.

Of course, any changes to the world file should retain portage's format, 
otherwise who knows what will happen.

If you know what you are doing, or are under the guidance of one who does, 
editing your world file is fine.
