As said, if I knew something about it I would tell.
It happened with real world traffic, with random WMS requests, some aligned
to the grid, many not, direct wms integration seemed to make the problem
more likely to happen, disabling it it was happening less frequently, but
in the end we had to remove GWC altogheter.
