It seems your data directory got corrupted, you have the
tasmania_water_bodies layer
stored multiple times in it. Not sure how could that happen, did you
perform manual
changes in the data dir folders?

One way to recover from that is probably to go into the data
directory, locate the
duplicated layer under "workspaces", and remove its configuration files and
folders manually
