Have been able to replicate this issue on our instance of Geoserver 2.1.1 with an ExternalGraphic. Rendering the WMS output as image/png, the ExternalGraphic shows up. However, rendering the WMS output as application/pdf, the ExternalGraphic is ignored. Sounds like a bug with GeoServer, not something particularly related to our SLDs, unfortunately.
