Have been able to replicate this issue on our instance of Geoserver 2.1.1 with 
an ExternalGraphic. Rendering the WMS output as image/png, the ExternalGraphic 
shows up. However, rendering the WMS output as application/pdf, the 
ExternalGraphic is ignored. Sounds like a bug with GeoServer, not something 
particularly related to our SLDs, unfortunately.

 I will keep waiting for a while if someone in the list would have  hands-on 
experience in that particular problem. If not, we'll choose our  way, whether 
it'd be a bug fix or upgrading or some other solution, and  proceed our journey 
along with that.
