Using 2 Levels results in Nullpointer Exception

The ImageMosaicReader has the same problem as ImagePyramidReader and
ImagePyramidFormat.
It will run into a NullpointerException when using 2 Levels. I am not sure if
that will ever happen with ImageMosaic, but it would be better to be robust for
input data, so I've fixed it. I'll attach the patch against 2.4-branch.
