Good morning:

There is no special procedure, each organisation needs to sign the contributors 
license covering their staff members. If the individuals are participating on 
their own time they should probably sign an individual contributors license as 
well.

Note that the two organisations probably had an arrangement in place to allow 
the collaborative effort to take place. This is usually such a bother that 
groups participate in projects like GeoTools rather then figure out how to do 
it with their respective legal departments.

#7 is usually for code that you found elsewhere and are repurposing. Check out 
the license page in the docs for some examples in the codebase. An easy to 
understand example is that for DateUtils noted on the page for the gt-main 
module.
