Well, the above link is clearly outdated. I will try to update it when I will 
have a chance.

The last step is to create the following text file:

META-INF/services/org.geotools.referencing.operation.MathTransformProvider

and put the classname of your projection in this file (one line for each 
implementation).

Example:

http://svn.geotools.org/geotools/trunk/gt/modules/library/referencing/src/main/resources/META-INF/services/org.geotools.referencing.operation.MathTransformProvider
