fixes #13800. when `set-destination URL` runs earlier in a remap rule, it replaces the request URL and sets `res.changed_url = true`. `OperatorRMDestination::exec()` was still using `res._rri->requestUrl` (the old handle), which caused a fatal crash in `traffic_server`. this change checks `if (res._rri && !res.changed_url)` and otherwise falls back to `TSHttpHdrUrlGet`, like `OperatorSetDestination::exec()` does (#12278). - tested locally (`traffic_server -K -k -R 1` and the Catch2 unit tests pass) - formatted with `clang-format 18.1.8` ### ai disclosure Assisted-by: antigravity:gemini. i used an ai assistant to trace the old handle in #13800, to follow the approach of #12278, and to check with clang-format and the regression tests. -- This is an automated message from the Apache Git Service.
