thanks for the thorough verification and for providing the uranium
regression test @bryancall!
i have:
1. added the uranium regression test in
`tests/uranium_tests/pluginTest/header_rewrite/header_rewrite_rm_destination.test.yaml`
with the apache license header.
2. updated the comment wording in `operators.cc` as suggested.
regarding `%{URL:...}` in `conditions.cc` and `set-redirect` in
`operators.cc`: i will look into those and follow up with a dedicated pr so
`set-redirect` can be handled with the extra care it requires.
