Each transaction took its reference to the global remap table through
`AtomicSharedPtr::load()`, which writes the same lock bit and control-block use
count from every thread, so it contends under load. This adds
`ThreadLeasedSharedPtr`, which hands out references through a per-thread
control block that's revalidated against a generation counter changed only on
reload, and uses it for the remap table in `HttpSM`. Replaced tables are still
freed when their last transaction finishes, as before.

Testing: new `test_ThreadLeasedSharedPtr` cases; pass on Linux and macOS.
