Right - I haven't released it properly because the documentation still
needs updating.

Sorry about that, It currently doesn't install properly from the
fptools tree, so that's why it didn't end up in the binary
distribution. This will be fixed for the next release.

See point 1. To see how to count line numbers in happy 1.3, check out
the example monadtest.ly in the examples directory.
