Do you *have* astigmatism, or need bifocals?  If not, then those other
values are zeroes, and can be left off.

Most of my prescription is astigmatism...the same is not true for
everyone.  Your "half-a**ed excuse" for a prescription may be totally
complete, except for the PD value.

I would call/visit the doctor and confirm the values.
