The Add will apply to both eyes. If you want readers then sum the
sphere and add values to get the reading sphere. If you need distance
then use the sphere, cylinder and axes values given on the
prescription, ignoring the Add. If you want progressive then provide
the sphere, cyl and axes values for each eye and the Add value.
Progressive lenses are very intolerant of poor lens location. Looking
straight ahead should use the distance portion of the lens, looking
down uses the reading part. In between the two there is a narrow
"corridor" that progressively moves between them. If this corridor is
not right in front of the eye then the wearer sees a distance lens, an
area of blur and then a reading lens, which is pretty hard to adjust
too. Also if the lens is to high in the frame then the wearer will
only be able to see into the distance by tilting their head down and
looking through the top portion of the lens. Great for plane spotters
but not for anyone else!
