Hi Dennis,
    Just a guess, but I'd say that the top nut on the pump was not
properly torqued at the last annual.
If I remember correctly they are not safey wired either, is that right?  I
feel stupid, but I don't
remember having wire on mine. My AP did that portion of the annual so
since I didn't do it myself I
don't remember.
