Did you not set up a trusted computer as well?  Did you consider printing
out your backup login codes?  Can you get your carrier to temporarily send
you the texts headed to your old number?

implies you could fill out that recovery page without the account set up
date "Take time to answer each question to the best of your ability"; since
all your other things will be right.

If you have access to the account from the phone, look for the first
message they ever put in your mailbox... that will be the day you opened
your account.
