at first sight, can you check your geometry (triangles) around points 2804 and 2667 ? The lines 16488 and 100000063 are also crossing each other. Idem for lines 18341 and 100000064, and there are more of them. As you can see, the cylinder is represented by small triangular faces, since they are the results of finite element calculations, during which the cylinder moves in time.
