You can use Gmsh to do global adaptation, or remeshing. I don't think it would 
be easy to incorporate MAdLib or some kind of local adaptation into Gmsh, 
despite the fact that MAdLib uses many Gmsh-based structures internally.

Mesh adaptation is a very different feature from mesh generation. I believe 
that adding this feature to Gmsh would further increase the power of this

You can integrate and use both gmsh and MAdLib on a C++ program, for example.
