I'm particularly interested in comments about the survey section. Anything
I should add to either section? Some additional questions we might want to
ask? Remove some questions or items?
Also, any spelling errors or typos I missed... :)
Note that the form doesn't actually go anywhere (yet), so clicking the
"Submit" button will just reload the page.
