DAMN, Ben that looks GOOD!!!
My only suggestion would be a comment block where the people could list any
suggestion that they are interested in seeing. Like " I use Peachtree
Accounting software for my business. Does this run under Linux or is there a
Linux equivalent??" This way we could see a broader picture of what they
are looking for without having to list too many check boxes.

I'm particularly interested in comments about the survey section. Anything
I should add to either section? Some additional questions we might want to
ask? Remove some questions or items?
