I used to be a manager and I am glad I got out of that track. The way
things work is that the company defines several job positions (or pay
grades) and titles. Each job position carries with it a salary range. It is up
to the company to decide what the titles are and also what the job
requirements are. Usually these are done by HR. For example, in the
military:
Army/AF/Marines Navy
01 == 2nd Lt. Ensign
02 == 1st Lt. Lt. JG
03 == Capt. Lt.
04 == Major Lt. Commander
Many times the titles are inflated to make people feel good. In a bank,
almost everyone is a vice president (assistant VP, VP, senior VP, ...).
Larger companies, like Raytheon, are very serious on what titles they
give out. I have seen them demote people because they did not meet the
requirements for that title. A principal engineer migh be required to have
been published and may be required to hold a certain degree. In other
words, a senior person in one company might be just a regular on
another.
