  In order:  No.  Yes.  :-)

  Assuming it isn't something stupid like a loose connection between the drive
and the IDE controller, the drive is failing and should be replaced.  Modern
hard drives have ECC built in to the hardware, so if you're seeing errors at
the OS level, the drive is so far gone the built-in ECC is overwhelmed.  Blah
blah blah, replace the drive.  :-)
