Hmm. \par almost certainly means that there's a stray empty line in the
ex input.

Well, it works fine over here, so you'll have to investigate a bit:

* check the .ly file:

    23:10:21 appel ~/usr/src/lilypond$ cat
Documentation/tex/out-www/reference-manual-1.6.2.ly

    % Generated by mudela-book.py; options are center fragment singleline verbatim
    \include "paper16.ly"
    \paper  { linewidth = -1.000000 \pt; }
    \score {
      \notes {
      ces' des' es' fes' ges' as' bes'
     }
      \paper { }

* if that's fine, run lilypond

    23:16:52 appel ~/usr/src/lilypond$ ./lily/out/lilypond
Documentation/tex/out-www/reference-manual-1.6.2.ly
GNU LilyPond 1.2.6.
    [..]
    Paper output to reference-manual-1.6.2.tex...
    [/home/fred/usr/src/lilypond/ps/lily.ps
    Line ... [0]

* and tex

    23:18:41 appel ~/usr/src/lilypond$ tex reference-manual-1.6.2.tex
    This is TeX, Version 3.14159 (Web2C 7.3)
    (reference-manual-1.6.2.tex (/home/fred/usr/src/lilypond/tex/lilyponddefs.tex
    (/home/fred/usr/src/lilypond/tex/lilypond-plaintex.tex
    LilyPond Plain TeX settings) (/home/fred/usr/src/lilypond/mf/out/feta20.tex)
    (/home/fred/usr/src/lilypond/tex/lily-ps-defs.tex) [footer empty]
    (/home/fred/usr/src/lilypond/tex/fetdefs.tex)) [1] )
    Output written on reference-manual-1.6.2.dvi (1 page, 3816 bytes).
    Transcript written on reference-manual-1.6.2.log.

by hand.

Now see what xdvi, or gv (after dvips) say.
