"CAVE"... caveat i.e. beware / caution ? (kind of like your precautionary notification to the patient's record in your example of leukopenia)
Aside to Richard: would you intend items in the scratchpad to be able to be seen/shared by more than one doctor? This would represent an alternative method (to what Karsten proposed) for how to let people be aware of something that may not need to be incorporated into main part of the record. Even so, there is value in supporting both.
OR did you intend it to be "only for the eyes" of the originating doctor?
