For this, you have to duly fill in Form No.6 and submit it to the Designated Officer at your Polling Stations or to the Electoral Registration Officer or Assistant Electoral Registration Officer of your Assembly Constituency upto 08th December 2003.
Copies of Form No. 6 are available at all Polling Stations.
