There is already a lot of validation done on the output file if I'm not mistaken (Looks for "Case #"). It is not really possible for someone to simply "swap" the output and the code. Those unknown submissions would have had a proper file for their output just the code is a wrong upload.
I'm more of the opinion that the instructions are simple. Upload the output file and your code. If you can't follow those simple instructions.
