There is already a lot of validation done on the output file if I'm not 
mistaken (Looks for "Case #").  It is not really possible for someone to simply 
"swap" the output and the code.  Those unknown submissions would have had a 
proper file for their output just the code is a wrong upload.

I'm more of the opinion that the instructions are simple.  Upload the output 
file and your code.  If you can't follow those simple instructions.  If one can 
solve these problems they should be able to follow those instructions.
