On mine apps it is working according to docs. in header gdata-version: 3.0 in URL v=3.0
Sorry but with the second part i do not help you if you need so specyfic info. I agree with you that there is lack of info in documentation but maybe this would help you. http://code.google.com/intl/pl/apis/contacts/docs/3.0/reference.html#ContactKindExtensions there are example of every entry.
