FYI - That form can only be used from a page hosted on google.com, so even
if it was within the Terms to host it yourself, it's not technically
feasible.
As for a Czech translation of that page, we don't have immediate plans for
doing one. You could suggest to your users that they run it through Google
translate before agreeing, or put a brief description of it on your site.
