I reckon it's best to do this server-side, so that your persona-non-
grata users never get the opportunity to register because the page
they are sent doesn't include the "Register" button. If you do your
validation client-side then it's easily circumvented and you will need
to re-validate server-side anyway.

There are a number of server-side solutions available. I've used data
from ip2location before when I wanted to avoid passing personal data
outside the European Union.

No IP-locating service is foolproof. For example, an AOL user in
Pakistan (to take a country more-or-less at random) is likely to
appear to be in the United States. And as Marcelo says, a badly-
behaved proxy will not use X-Forwarded-For and you won't know where
the end user is anyway.
