Why can you not include a link, as requested, instead of a code
listing which is specifically NOT advised in the posting guidelines?

A disabled maps page doesn't show what's going wrong. It might also
break the Terms of Service -- do they require a map?

Why not add a page which doesn't work, unlinked to anything else on
your website so visitors don't stumble upon it by accident, and
include a link to that here? Then we can see and debug what's going
wrong. No-one can run a debugger on an incomplete code listing.
