As a follow up:

If a user doesn't currently have a Google Health profile, they'll get
the Terms of
Service before hitting the AuthSub profile linking page (http://
code.google.com/apis/health/images/3pdoc-authsub.jpg).
A profile will automatically be created for them after accepting the
terms.

If the user already has one or more profiles, they'll just
get the linking page when your app sends them to AuthSub.

So there's no need to create profiles on the fly :)
