Try going commando in the Terminal. Type,
gksu displayconfig-gtk

Make sure that the second monitor in this dialog is enabled and set to
the right model, resolution, and hertz. The dialog is pretty self
explanatory and has a listing of hundreds of monitors all organized
easily by manufacturer and alphabetical. If you can't find your type
just select Generic Plug n' Play for an older monitor or LCD Panel.
