Encounter 123 illustrates the capture of PRO using Short Form-36. In addition to the concepts being unique to the form used to capture the information there is the additional inclusion of the provider_id indicating a patient reported outcome by Short Form-36 Encounter 456 illustrates the capture of PRO using MyChart. The concepts are not unique to MyChart since the information entered in MyChart is integrated with the EMR. Instead we use the provider_id to indicate a patient reported outcome using MyChart.
