Yes, we added a question to our survey: "By marking YES, you are agreeing
to participate in this study. If you do not want to participate in this
study, click NO, and the survey will close"
I'm not sure how we got the survey to click close if they click NO.

We also put a blurb in the invitation text:
"3. If you do not wish to participate in this survey, please open the
survey and select NO to the first question."
