This is the same search your institutions are using for the ADAPTABLE recruitment. While recruitment happens at the local level, Dr. Rosenthal would like to know how many patients meet the search criteria at your sites. The query is located on Babel at Shared\ADAPTABLE\ADAPTABLE_WO_NCDR_04182016. Attached is a description of the inclusion and exclusion criteria.
