 I found most of the specifics on the REDCap survey. For Marshfield, I created 
a zip file with two csvs - one for diagnoses and one for procedures.

If these have been reviewed or require revision, I haven't heard it yet...

Hi,

I need a bit of direction.

Per the GPC meeting discussion on obtaining information on 
procedures,conditions,diagnoses from the CDMV3 table for consented patients in 
the breast cancer cohort ...

UNMC personnel have extracted this information into an Excel spreadsheet with 3 
sheets for procedures, conditions, diagnoses.  However, I am uncertain of the 
format that is being requested (is an xlsx spreadsheet file good enough?) and 
how to go about submitting the information.  The only thing I know is that it 
is to be sent to Betsy, and I am not sure how to go about that.

Can anyone point me to appropriate information?  I have not found the 
information in ticket #448.
