Let's try collecting these via GPC REDCap as usual; note the use of real dates:

survey: diagnoses and procedures for consented Breast Cancer Study
patients<https://redcap.gpcnetwork.org/surveys/?s=EEF8DKCA4T>
and in the survey we see:
Make a .zip archive of .csv files and attach it.
A spreadsheet with several sheets may be close enough for Brad McDowell and
Betsy C. and company; I don't know.

Be sure to specify which CDM data goes with which breast cancer study ids.
UIOWA and KUMC used a crosswalk from patid to study_id. MCRF added a column to
their CDM tables.

Subject: UNMC Breast Cancer cohort consented patients -- how to send CDMV3 info
for procedures,etc?

Hi,

I need a bit of direction.

Per the GPC meeting discussion on obtaining information on
procedures, conditions, diagnoses from the CDMV3 table for consented patients in
the breast cancer cohort ...

UNMC personnel have extracted this information into an Excel spreadsheet with 3
sheets for procedures, conditions, diagnoses. However, I am uncertain of the
format that is being requested (is an xlsx spreadsheet file good enough?) and
how to go about submitting the information. The only thing I know is that it
is to be sent to Betsy, and I am not sure how to go about that.

Can anyone point me to appropriate information? I have not found the
information in ticket #448.
