qt_query_instance does have a PK of query_instance_id so I would expect
the insert to include this non nullable field :
insert into qt_query_instance (query_instance_id, user_id, group_id,
patient_set, start_date, status_type_id, message) values (
QT_SQ_QI_QIID.NEXTVAL , etc.... )
We're not in Mysql land toto ...
