Or Totalcare, seperate databases for billing / demographics; clinical and letters / attachments (which are actually stored in the file system but have their own management database). It's so clever that you can have one database on SQLBASE, one on MSSQL, one on PostgreSQL, etc. Transparent to the user, although the applications are also seperate modular executables, Totalcare, Lettercare, Totalclaims, Caremanager, Totalcoms, Bankcare, MUSE.
