I agree with all this Duncan, but money talks. It seems as if the pattern is to create a successful organisation, achieve efficiencies through modern business techniques (viz. don't let the docs run the show) and then wait for the next stock market boom and sell at some fantastic multiple. The docs don't seem to have quite worked out what to do after that, however.
