Indeed, and while we're at it, why not print a Australia Post-style "2D" 
barcode on the referral/request form,
which contains the same data, saving data entry (which is the aim of the game)
The barcode can also contain a web address and a one-time password to securely 
submit the result back to the GP's web service.
