I'm a late starter but have put about 20 sets through now
Adjectives are often missed where really appropriate like 'inguinal hernia' is 
much more meaningful than 'hernia'. 
While the system knows Hb it doesn't know FBC (of which Hb is a subset)
nor BSL nor WEIGHT although it knows BMI.
I've submitted all of these comments through the system, of course 
but so far it isn't going to get my stuff coded, even where i have made sure 
that the diagnosis appears in the text (a 'reason for visit' doesn't get 
copied over by me) the diagnosis can be miscoded by losing an adjective (like 
inguinal)
