Greg has kindly sent me this link in which Michael Kidd and the RACGP
summarised the state of play in mid 2005.

It is illuminating reading.

It looks like the most advanced groups are E-Link Central Coast,
possibly SADI and perhaps the most successful is Peninsula Health.

The two key themes to succeeding are:-

   1. rendering the discharge documents in text
   2. having someone driving it on the hospital side

The Contrasting Views Comment 3 suggests that Commonwealth funds may be
accessed through GP participation in a discharge care plan and claiming
and Item 729. Would that work?
