It looks like the most advanced groups are E-Link Central Coast, possibly SADI and perhaps the most successful is Peninsula Health. The two key themes to succeeding are:- 1. rendering the discharge documents in text 2. having someone driving it on the hospital side The Contrasting Views Comment 3 suggests that Commonwealth funds may be accessed through GP participation in a discharge care plan and claiming and Item 729.
