Well you know where I sit in this argument, but I'm too tired to bother.
Those few sites where I have a choice over the OS (and where the
chosen medical software demands Windows (due solely to short-sighted
developers)) run a single Windows machine with a single anti-virus
license, no matter how big they are.
In this age of terminal services and application publishing, you
wouldn't catch me running multiple Windows machines, it's not worth
the pain.
