If it's just the demographic data you want, then PracSoft can dump
this data to PATIENTS.IN, a big text file with a standard format, and
then it can be easily imported back into a new program.
Unfortunately MD3/MedTech have no similar functionality for clinical notes.

I did get the impression from this thread that some people wanted to whack
a web-interface on top of a SQLised PracSoft schema. Whatever the technical 
rights and wrongs of this, HCN could make a pretty convincing case of 
copyright violation.
Converting to a 'cleanroom' schema (i.e. one designed by people who have never
seen inside a HCN database) is pretty safe, as you can argue that the 
inevitable similarities are co-incidence.
