These labels must also be placed on the same line, as shown below: Must have: Patient Name: <<patient name>> DOB: <<DOB>> Choose one Referred by: <<doctor>> Date of Report: <<date>> of these: Recipient: <<doctor>> Date of Report: <<date>> These lines do not need to be in this order, the lines can appear anywhere in the document as long as these fields appear on the same line as shown above.
