This is what I am trying to understand. I see that the authors are a respiratory and sleep physician and a consultant rheumatologist in a public hospital in Brisbane. Is it the business or perhaps just in the career interest of public hospital specialists to be looking for zebras rather than any other more common equine when hoofbeats are heard, perhaps on the basis that we GPs and specialists in private practice have already identified most of the horses and donkeys that are galloping about the place?

I am waiting to hear back from Dr. Hangwi Tang, the corresponding author, about how they worked out their search terms.
