You comply with the PBS rules if you try a sample steroid inhaler first and if 
don't get a satisfactory result, you can prescribe the combo on PBS

However, I usually do it the other way around -I give them a combined inhaler 
sample when they present poorly controlled, and once the symptoms seem 
controlled I start downtitration with just plain steroid inhalers per script.

In some this doesn't achieve good enough control, and they are the ones who 
get a script for the combo
